Media Acknowledgement

The following is a media acknowledgement for Cross Surface Hockey Development Ltd. (CSHD). CSHD and people acting on behalf of CSHD will actively take photos and videos of participants during the association’s publicly held events. The associations, or people acting on behalf of the associations will and may interview, photograph, videography, and / or audio record participants and spectators to use and edit, without compensation to myself or the participant listed on this application, for the purpose of media and / or advertising on CSHD’s social media, website, brochures, and other platforms, and for the ultimate purpose of promoting the sport of ball and ice hockey development with the community.

 

Note: CSHD takes personal information very seriously, especially for youth participants. No use of names or other personal information will be used without the explicit and written consent of the participant's legal guardian(s). This acknowledgement gives consent only for the use of visual content.
